What causes this issue?

As the approval agent, you have approved the status change &V1&.

System Response

If applicable, the status change will be submitted for approval to other
approval agents. If no other approvals are required, the system changes
the status of the corresponding measure to &V2&" and ends the approval
workflow.
It is possible that this process has been changed through a BAdI
implementation. In this case, despite the approval, the system may not
immediately carry out the status change or may change the status of the
task to "&V3&" even though the status change has been approved.

How to fix this error?

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.
