Online Service, in ESS, which is valid for the period between &V1& and
&V2&, for &V3& &V4&,
however, no form was made available for this employee via Online
Service.
A possible reason is that the employee was terminated between the date
in which the consent was given and the W-2 production run date in Tax
Reporter.
