    How to fix this error?

    Please enter an outline paragraph which has already been defined. The
    paragraph itself is entered as the "highest-level" outline paragraph of
    an outline hierarchy. You can define the individual paragraphs of the
    outline hierarchy in descending order only, first the "highest-level"
    paragraph, then the next, etc.
    Example: An outline comprises three paragraphs, AA,BB, and CC, where AA
    is the highest and CC the lowest paragraph in the hierarchy. The
    paragraphs must then be created in the order AA,BB,CC, where the
    paragraph AA is entered as the outline paragraph for all three
    paragraphs.
