What causes this issue?

An incorrect outline level was specified for an outline paragraph.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

The outline level of the "highest-level" paragraph of an outline
hierarchy must always be 1. All lower outline paragraphs are numbered
upwards from 1 (2, 3, etc.). If a new outline paragraph is created, it
must have the next free outline level. Please enter the value specified
in the message as the outline level for the paragraph.
