What causes this issue?

The outline level of the paragraph is incorrect.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

Enter the next free outline level of this outline as the outline level
of the paragraph.
Example: The form already contains the outline paragraphs AA and BB,
where AA is the "highest" paragraph in the outline hierarchy and BB the
"second highest". AA is the outline paragraph for AA, and AA is also
specified as the outline paragraph for BB. The outline level of AA is 1
and BB 2.
If a new outline paragraph CC is now created (for the outline AA), it
will have outline level 3.
