What causes this issue?

1 was not specified as the outline level for the "highest-level"
paragraph in the outline hierarchy.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

The outline level of the "highest-level" paragraph in an outline
hierarchy must always be 1. All lower outline paragraphs are numbered
upwards from there, 2, 3, etc. Please enter the value 1 for the outline
level.
