  • What causes this issue?

The infotype was inserted in the record.
<ZK>Anchor</> is an attribute of model nodes in the record model
(attribute ANCHOR). Values of this attribute act as markers for
elements that the system automatically enters in the record.
If you have created a model node for each infotype in the record model,
which has the anchor: <Value entered in the input template><Infotype
number>, the infotype is inserted for the model node with the
    corresponding infotype number.
If you have created a model node in the record model, whose anchor is
the same as the anchor entered in the input template of the report, the
infotype is entered at this anchor.
If you have not created a suitable model node, the infotype is entered
at the anchor * of the model node <ZK>Worklist element</>. This is
entered by default as the top node in every record model.
