  1. Enter the transaction code F1434 in the command field. 
  2. Select the “Create” option from the menu bar. 
  3. Enter a name for the form template in the “Form Template Name” field. 
  4. Select a document type from the drop-down list in the “Document Type” field. 
  5. Select a language from the drop-down list in the “Language” field. 
  6. Select a layout from the drop-down list in the “Layout” field. 
  7. Enter a description for the form template in the “Description” field (optional). 
  8. Click on “Save” to save your changes.
