You want a structure to be generated for an entity type with
<DS:DE.USMD_USAGE_TYPE>storage and use type</> <LS>4</>. However, the
structure for the higher-level entity type &V3& with storage and use
type <LS>1</> is not generated (for using <LS>PDF-based forms</>).
