You tried to set up an office template for the COBRA letter and a
SAPScript template for the COBRA enrollment form, or vice versa.
However, COBRA enrollment can only be processed if the COBRA letter and
enrollment form are both either office or SAPScript templates. It is
not possible to set up a mixed configuration of office and SAPScript.
