¿Qué causa este problema?

An error occurred when updating a log entry for the transaction. The
full key for the problematical entry is:
&V2&/&V3&
The return code for the database is: &V4& (4 = entry was already in the
database, 8 = entry could not be written).

Respuesta del sistema

No entry could be written. The entire transaction is reset (ROLLBACK).

¿Cómo arreglar este error?

El sistema emite un mensaje de error y no le permitirá continuar con esta transacción hasta que se resuelva el error.
