exception program &v2& are combined in a schema step, since they occur

during billing</> is not identical.

Exclude one of the TOU exceptions from billing by entering a TOU
exception response with the <LS>Opt-Out</> category for one of the TOU
exceptions.
Change the periods for the TOU exceptions so that they do not overlap.
