You have checked more than one box within one plan type.

Even though the employee may have the choice of several plans of a
certain type, e.g. dental, enrollment is only allowed in one plan, e.g.
Dento Plan.
Also, within one plan, you may select only one option.

¿Cómo arreglar este error?

Double check that for each plan type, you have only checked
one box.
