What causes this issue?

The addition of a partner relationship previously announced was not
carried out, or was undone.

System Response

The automatically added partner relationship was deleted / is no longer
assigned to the RP account. Since this took place during processing or
during user-questioning, it cannot be reproduced onscreen.

How to fix this error?

Check your decision and the partner relationships assigned to the RP
account.
