What causes this issue?

The system tried to initialize the SAP 3D Visual Enterprise Structure
Manager, but was unsuccessful. There could be multiple reasons for this.

+,,The user defined SAP GUI Security Settings rules are configured in a
way to deny the ?Execute? access to the SAP 3D Visual Enterprise
Structure Manager ActiveX control. This could happen if you have
selected ?Deny? and checked ?Remember my decision? in the SAP GUI
Security dialog during your previous attempts to launch SAP 3D Visual
Enterprise Structure Manager.

In the SAP GUI Security dialog, always click ?Allow?. You can also check
?Remember my decision? to avoid getting this dialog every time the SAP
3D Visual Enterprise Structure Manager is launched.
If you do not get the SAP GUI Security dialog upon attempting to launch
SAP 3D Visual Enterprise Structure Manager, the SAP GUI Security
Settings would have to be changed to provide ?Execute? access to the SAP
3D Visual Enterprise Structure Manager ActiveX control. To do this,
follow the steps outlined below.
+,,Open SAP GUI Options Dialog (Press Alt + F12 and select ?Options?
from the menu).
+,,Select Security - Security Settings from the options on the left of
the dialog.
+,,Click on ?Open Security Configuration? button.
+,,In the list of rules, check the rule for object
?SAP.VE.VSM.VsmServerWrapper.1?. Ensure that this rule has ?Action? as
