What causes this issue?

Entity &V2& of entity type &V1& cannot be deleted because entity &V2& is
still in the hierarchy of entity type &V3&.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

First remove all the places where entity &V2& of entity type &V1& is in
the hierarchy of entity type &V3&. You can then delete the entity.
