What causes this issue?

In a hierarchy, an entity can only be represented by one direct child
node of a parent node. This is regardless of whether the child node is a
single node or a range node. You have tried to insert entity &V1& &V2&
under parent node &V3& &V4& that is already covered by a range node.

System Response

The entity is not inserted.

How to fix this error?

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.
