What causes this issue?

In a hierarchy, an entity can only be inserted directly under the parent
node once. However, entity &V1& &V2& has already been inserted directly
under &V3& &V4&.

System Response

The entity is not inserted again.

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.
