What causes this issue?

When saving the user settings to the database, an error occured for an
INSERT.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

Try saving the settings again.
If this does not fix the problem, delete your settings using the
report RSTPDAADMIN and then try saving again.
