What causes this issue?

All changes and deletions made to terminology entries and selected
attributes are automatically logged in the form of change documents.
An error occurred during the creation of these change documents.

System Response

The R/3 System did not create any change documents. This will cause
gaps to appear in the change log.

How to fix this error?

Report the problem immediately with a CSS message using component
CA-DOC-TER.
