What causes this issue?

An entity with the name "&V1&" and the entity type &V2& (L = local
entity, E = external entity, A = affiliation) already exists.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

Use another name.
