What causes this issue?

The sub-operation has already been confirmed manually, that is, data
was entered on the detail screens. Any subsequent confirmations must
therefore also be entered on the sub-operation detail screens.
This means that you can no longer create a confirmation for the
operation from the list of sub-operations.
