What causes this issue?

A node whose key already occurs at another location was inserted into
the structure. Structure entries with identical keys are assigned the
same title.

System Response

The existing title was used.

How to fix this error?

If you assigned the keys twice by accident, change the keys. You can
then assign your own title.
